Across Oxfordshire some GP practices are already providing an enhanced service to those people with diabetes who have very high blood sugar levels (HbA1c over 10). Reducing sugar levels can help to reduce the risk of blindness and toe problems leading to amputation. This enhanced service will include the opportunity to identify goals, which if they are acheived will help sufferers to have more independence and control over their condition.
