Diabetes2gether is an education programme for newly diagnosed Type II diabetics and is a vital and essential part of routine care for people within Oxfordshire with diabetes. It is offered to all, but work is being undertaken to ensure that equity and diversity to all groups is achieved.

Patients are invited to attend a three hour structured education programme covering

  • What is Diabetes?
  • Healthy eating
  • Treatment options
  • Self management plan

It is run on an interactive basis, so enhancing the learning experience and addressing the individual needs expressed by the patients. You can access Diabetes2gether either by

Katie Seal and Diane Boscott are the lead educators on this programme and have a team, of 14 educators who also deliver the programme who are either nurses or dietitians.

 

BME Structured Education Programme for Type II Diabetes

This training is targeting patients suffering Type Two diabetes fall under the following category;

  • For whom English is not their first language
  • Where cultural sensitivity is paramount
  • Newly diagnosed patients
  • Those who are not insulin dependent
  • Those who have had diabetes for less than 4 years

The Health Advocacy Service is supporting the training by:

  • By raising the profile of the training to GPs and health care practitioners
  • Coordinating the list of patients who will be attending the training
  • Raising the profile of the training to the community
