Wound Care (Tissue Viability)
Introduction
The Tissue Viability Service (Wound Care) is a nurse-led service which provides specialist advice and care to patients with, or at risk of, developing wounds. This is achieved by the provision of specialist advice, training & equipment.
What does our service provide?
- Provide high quality information and evidence based care that will contribute to realistic and achievable clinical outcomes.
- Work in partnership with patients, their carers and healthcare professionals to provide expert wound care advice, equipment and the promotion of maintaining intact skin.
- Develop and implement guidelines and policies to ensure best practice.
- Conduct audit and evaluations within the service with an aim to improve practice.
- Provide training and education to Clinicians providing wound care as part of their role.
Who is the service for?
The service is to aid and support all healthcare professionals who are involved in the management of:
- Complex wounds e.g. pressure ulcers, surgical wounds
- Leg ulcers and Lymphoedema
- Equipment needs of wound care patients
Hours of Service
Monday – Friday 8.30 am – 4.30 pm
Equipment request ‘cut off’ fax times for
- Same day delivery is 14.30hrs
- Next day delivery is 15.30hrs
Any phonecalls/answerphone messages received after 16.00hrs will not be actioned until the following working day.
How can a Referral be made?
Patients can be referred by their GP, Community nurse or any other health care professional.

We are based in the Administration Building, Abingdon Community Hospital, Abingdon OX14 1AG and we cover the whole of Oxfordshire community including:
- GP surgeries
- Care homes
- Community nursing teams (Patients own homes)
- Community hospitals
