YES I want one	NO thanks	Fill out a combined opt-out form for the Summary Care Record and Oxfordshire Care Summary. You can also phone 01865 428886, or email health.records@oxfordshirepct.nhs.uk for a form to be sent to you. You can also pick up a combined opt-out form from your GP Surgery.

NO thanks	NO thanks	Fill out an opt-out form for the national Summary Care Record - this will automatically mean that you will NOT have a local Oxfordshire Care Summary too. The opt-out form will be mailed to ALL residents in Oxfordshire at the beginning of April 2012. Additional opt-out forms are available to download here.

As a parent or guardian, you are responsible for your child's choice or should support them in making their own decision if you feel they are old enough to do this.

Whatever you decide, the NHS will always continue to do its best to provide you with safe, efficient care.

What if I change my mind?

If you have said no to a Summary Care Record and/or Oxfordshire Care Summary but then change your mind, we can still create one for you.
If you have said yes to a Summary Care Record and/or Oxfordshire Care Summary but then decide you do not want it, we will 'hide and lock' your record to make sure that healthcare staff who try to access it will not be able to see it.
